Medical summaries form the essence for resolution of personal injury cases. It describes the accident, the mechanism of injury and the immediate care provided at the scene and in the emergency room in a summarized form. Records of hospital stay, subsequent treating physicians and diagnostic testing are summarized. Treating physicians' comments regarding causation of the injuries, prognosis and permanency are included in the summary.
The medical records are first sorted and organized in chronological order, starting with the first day of the admission and the pages are bate numbered. The analysis of medical issues begins with a careful review of the records. The client's chief complaint, history of present illness, past medical history, surgical history, family history, social history (including smoking, alcohol, and drug abuse), along with current medications and allergies are captured. We also mention the expected course of action to address the symptoms including follow up visits, medication (strength/ dosage/ frequency/ route), procedures, surgeries, therapies (frequency/ duration), chiropractic care, radiological and laboratory tests, referrals, off-work or off-school notes, restrictions, diet / nutrition, etc.
